About Medical Records from PACC: With the closure of our office in May 2014, medical records may be requested by mailing a completed copy of the records release form (or a records release form from your current doctor) to:

Records will be mailed to you or to your physician’s office, as requested. Please allow three weeks for delivery.
About Immunization Records: For most children, this is the single most important medical record, and usually the main item parents need to keep and bring with them when changing to a new doctor. For the last ten years our office used CIIS (Colorado Immunization Information System), the state sponsored system. ALL immunization data from our office is recorded in CIIS.
A copy of the last CIIS shot record will be included with each child’s records (but will not include vaccines recorded AFTER their most recent immunization at our office). Please note, however, that our school systems have access to view and print current CIIS records, as does Fremont County Nursing Services and many of the local physician’s offices. Finally, note that using your child’s school sponsored Parent Portal will give you access to the vaccine records held by the school. If you need immediate access to a shot record, these options may be faster.
About Janet: Janet Krein, PA-C, has accepted a position with the St. Thomas More Physician Group, and is now seeing patients. She is working with St. Thomas More’s new pediatrician, Dr. Elizabeth Wilson. Currently, they are seeing patients at 614 Yale Place. Appointments may be scheduled with Janet or Dr. Wilson by calling the STM Physician Group at 719-285-2700.
